Job summary

Sandhills Medical Foundation is seeking a Pharmacist to serve patients by preparing medications, providing pharmacological information to the care team, and monitoring drug therapies. The Pharmacist will be part of a PCMH team and supervise ancillary pharmacy staff.

Responsibilities include formulating medications per orders, assessing drug interactions, counseling patients, managing formulary and purchasing, and overseeing billing processes for covered and sliding fee services.

Qualifications

• Licensed Pharmacist in good standing with SC Board of Pharmacy.
• Pharmacist must participate with Medicaid, Medicare, and third-party payers.

Responsibilities

• Prepares medications by reviewing and interpreting provider orders.
• Oversees drug therapies and advises interventions as needed.
• Counsels patients on newMedication therapies and other medications as appropriate.
• Manages inventory and pharmacy pricing strategy consistent with SMF mission.
• Directs medication formulary management and purchasing systems.
• Handles billing and collects information for claims payments.

Position Overview:

The overall responsibility of the Pharmacist is to serve patients by preparing medications, giving pharmacological information to multidisciplinary health care teams, and monitoring patient drug therapies. In addition, the Pharmacist is part of a Patient-Centered Medical Home (PCMH) team and is expected to be familiar with the philosophy, goals, and objectives of the PCMH. II. Supervisory Responsibilities: Responsible for supervision of the ancillary pharmacy staff

Duties & Responsibilities:

• Prepares medications by reviewing and interpreting provider's order(s)
• Detects therapeutic incompatibilities
• Dispenses medications by compounding, packaging, and labeling pharmaceuticals
• Oversees medications by monitoring drug therapies and advises when interventions are appropriate
• Provides pharmacological information by answering questions and requests of health care providers
• Counsels patients on all new medication therapies, and other medications as seen appropriate
• Effectively and efficiently manages the pharmacy department's inventory and assets in a manner that is consistent with the mission of Sandhills Medical Foundation's (SMF) mission
• Develops and adjusts (as needed and approved) the pharmacy pricing strategy
• Directs the organization in the development and management of its medication formulary
• Manages/Provides oversight for the organization's medication purchasing system
• Oversees and makes provisions as appropriate for all billing services for covered and sliding fee services. This includes, but is not limited to, marking claims as paid as insurance payments are received and posted
• Directs, manages, oversees, and (as necessary) generates revenues by calculating, recording and issuing changes directly to patients by assisting with medication pick-up
• Maintains confidentiality and privacy in accordance with HIPAA and SMF policies
• Exhibits professional conduct at all times
• Performs all other duties as assigned

Required Education, Certifications, Skills & Abilities:

• Bachelor's Degree ln Pharmacy, Pharm D. Degree or equivalent FPGE (NABP)
• Licensed pharmacist in good standing with the South Carolina Board of Pharmacy
• Ability to participate with Medicaid, Medicare, and third-party payers SANDILLS MEDICAL FOUNDATION, INC.

Additional Requirements:

• Must be willing to work with SMF EMR system
• Must have a valid driver license
• Must be willing to work at different SMF locations if requested

Training Requirements:

• The Pharmacist will be responsible for attending all mandatory training including evidence based approaches to self-management support, proactively addressing the needs of patients and families in the community, cultural competency, effective communication, care coordination, OSHA, HIPAA, and EHR, etc.
• Shall complete annual CMS Fraud 1 Waste and Abuse training
• Shall complete fifteen hours (1.0 CEU's) of American Council on Pharmaceutical Education (ACPE) accredited continuing pharmacy education or continuing medical education (CME), Category I, or both, each license year. Of the fifteen hours, a minimum of 6 hours must be obtained through attendance at lectures, seminars, or workshops. At least fifty percent of the total number of hours required must be in drug therapy or patient management. VII.

• Ability to continuously stand and walk on carpet, tile and concrete the majority of the time
• Ability to occasionally move, carry, push, and lift objects up to 25 lbs. without assistance
• Ability to continuously reach overhead to access medications
• Ability to continuously perform repetitive movements.
