STGi is currently seeking a Clinical Pharmacist to provide services at a Community Based Outpatient Clinic.

Job Summary:

The Clinical Pharmacy Specialist (CPS) will focus on drug monitoring and management of therapy, with emphasis on diabetes, hypertension, and dyslipidemia. The CPS is an integral part of the team that is assigned to a panel of patients to support continuity of care. The CPS provides coordination and provision of clinical pharmacy services to the patients and staff at the clinic.

Essential Functions:
  1. The CPS functions at the highest level of clinical practice, working independently within their scope of practice to care for patients.
  2. Plays a role in budgetary execution and functions as a mid-level provider to initiate, modify, or discontinue medication therapy, and as a consultant for medication management services.
  3. Designs, implements, assesses, monitors, and documents therapeutic plans using effective and economical medication treatments.
  4. Helps achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams.
  5. Performs physical assessments.
  6. Orders laboratory and other tests to evaluate medication therapy efficacy and toxicity.
Required Skills:
  • Excellent communication skills to influence clinical and management decisions.
  • Expertise in regulatory and quality standards.
  • Problem-solving and organizational skills to maximize outcomes.
  • Specialized knowledge in clinical pharmacy practice or a pharmacy specialty.
  • Advanced skills in monitoring drug therapy outcomes, including physical assessments and interpretation of diagnostic parameters.
Required Education:

Graduate of an accredited Pharm.D program, preferably with an Ambulatory Care residency, or equivalent experience. Must have a degree from an ACPE-accredited College or School of Pharmacy.

Working Conditions/Physical Requirements:

Moderate physical activity; ability to handle objects up to 25 pounds; standing/walking for extended periods; typical office environment; respond quickly in medical emergencies.

Required Experience:
  • At least one year of recent clinical pharmacology provider experience.
  • Experience managing pharmaceutical care in outpatient settings preferred.
  • Unrestricted CA license as a Clinical Pharmacist; current CPR/BLS certification.
