Pay Range: $50.00 – $79.00/hour

Position Summary:
A respected healthcare system in Fort Wayne, Indiana is seeking a highly skilled and motivated Medical Doctor/Physician – Pharmacist to serve in a clinical generalist capacity. This role is ideal for a pharmacist with strong clinical experience who is passionate about delivering direct patient care and collaborating within interdisciplinary healthcare teams.
The individual will rotate between clinical and distribution functions, with over 60% of the role focused on direct clinical pharmacy services. Daily responsibilities include participation in multidisciplinary rounds, optimizing pharmacologic therapies, and ensuring safe and effective medication use across various patient populations.
Key Responsibilities:
- Deliver direct patient care through daily multidisciplinary rounding.
- Evaluate, monitor, and document medication therapies and clinical interventions.
- Prepare and dispense medications in accordance with state and federal regulations.
- Collaborate with physicians and healthcare staff to manage drug therapies and patient outcomes.
- Provide medication and patient education, and support safe medication use practices.
- Monitor and manage therapies such as anticoagulants, vancomycin, total parenteral nutrition (TPN), and patient-controlled analgesia (PCA).
- Precept and mentor pharmacy students (IPPE/APPE), PGY1 residents, and PGY2 ambulatory care residents.
- Participate in performance improvement initiatives and medication safety programs.
Qualifications:
- Doctor of Pharmacy (PharmD) or equivalent degree from an ACPE-accredited institution
- Completion of a PGY1 residency or equivalent inpatient/hospital pharmacy experience
- Eligible for pharmacist licensure in the state of Indiana
- Proven clinical decision-making and critical thinking skills
- Strong interest in teaching, mentoring, and contributing to clinical education
- Excellent communication and interpersonal collaboration skills
Compliance & Health Requirements:
- Proof of positive MMR (Measles, Mumps, Rubella) titers or documentation of two MMR vaccinations is required.
- If titers are negative, two MMR booster shots must be administered following the negative titer result.
