Job summary

Life Tree Pharmacy, a Merakey subsidiary, seeks a licensed Pharmacist to compound and dispense medications for patient care in a long-term care setting. The role includes reviewing prescriptions, directing staff, and maintaining inventory and documentation.

Two or more years of related LTC experience are preferred, with LTC ECM familiarity highly regarded. Full-time, on-site position offering comprehensive benefits and professional development opportunities.

Qualifications

• Must hold a current Pharmacist License in good standing.
• Requires 2+ years related experience and/or training.
• Framework LTC and Framework ECM experience preferred.
• 1 year of experience in LTC pharmacy preferred.

Responsibilities

• Reviews prescriptions issued by physicians or other authorized prescribers to ensure accuracy and determine formulas and ingredients.
• Compounds medications using standard formulas and processes, including weighing, measuring and mixing ingredients.
• Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
• Answers questions and provides information on drug interactions, side effects, dosage and storage of pharmaceuticals.
• Maintains established procedures for quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
• Enters data such as patient name, prescribed medication and cost to maintain pharmacy files, charge system, and inventory.
• Instructs interns and other medical personnel on matters pertaining to pharmacy.
• Employs perpetual inventory.

Position Details

Compounds and dispenses prescribed medications, drugs, and other pharmaceuticals for patient care by performing the following duties:

• Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
