Job summary

Crawford Thomas Recruiting is seeking a Pharmacist to provide clinical pharmacy services for patients receiving home infusion therapy in Columbus, OH.

You will oversee medication therapy, IV admixture, patient monitoring, education, and coordination of pharmacy services with physicians, nurses and other healthcare professionals.

Active Ohio Pharmacist license required; 2+ years acute care or 1+ year home infusion experience; salary $128,000–$135,200 with Day 1 benefits.

Qualifications

• Active Ohio Registered Pharmacist license required.
• 2+ years acute-care experience or 1+ year home infusion experience.
• Strong knowledge of IV admixture and intravenous therapy.
• Strong clinical judgment and medication-management skills.
• Ability to supervise pharmacy support staff.
• Strong communication, organization, and time-management skills.

Responsibilities

• Evaluate patients and prescribed therapies for home infusion care.

• Review labs, perform pharmacokinetic dosing, and make clinical recommendations.
• Dispense medications, supplies, and infusion equipment.
• Educate patients and caregivers on medications and infusion therapies.

• Ensure compounding and clean-room activities meet USP standards.
• Coordinate refills, supplies, labs, compounding, and deliveries.
