Crawford Thomas Recruiting is seeking a licensed Pharmacist for a Home Infusion role in Syracuse, NY to provide clinical pharmacy services for patients. You’ll oversee medication therapy, IV compounding, patient monitoring, education, and coordination with physicians, nurses and other healthcare professionals.

Requirements include Active New York Pharmacist License, BS in Pharmacy (PharmD or MS preferred), 2+ years acute-care OR 1+ year home infusion experience, and strong IV therapy knowledge.

Responsibilities

• Evaluate patients and prescribed therapies for home infusion care.

• Review labs, perform pharmacokinetic dosing, and make clinical recommendations.
• Dispense medications, supplies, and infusion equipment.
• Educate patients and caregivers on medications and infusion therapies.
• Monitor patient progress, compliance, labs, and treatment outcomes.
• Ensure compounding and clean-room activities meet USP standards.
• Coordinate refills, supplies, labs, compounding, and deliveries.
• Supervise and support pharmacy technicians and other pharmacy personnel.
• Participate in on-call responsibilities as required.

Position Overview

Our client is seeking a Pharmacist to provide clinical pharmacy services for patients receiving home infusion therapy.

You’ll oversee medication therapy, IV compounding, patient monitoring, education, and coordination of pharmacy services while collaborating with physicians, nurses, and other healthcare professionals.
