Job summary

Quadrant, Inc. is seeking a licensed Pharmacist in Anchorage, Alaska. The role requires PIC experience and may prefer candidates with Behavioral Health background.

The pharmacist will review medication therapy, perform chart reviews, and ensure proper dosing and documentation. Weekly on-site facility visits and monthly unit reviews are part of the duties, along with ensuring stock levels and coordinating with wholesalers for alternatives.

Qualifications

• Licensed as a Pharmacist in Alaska.
• Experience as Pharmacist in Charge (PIC) required.
• Behavioral Health experience preferred.

Responsibilities

• Review patient charts for medication therapy, allergies, interactions, selection, route, and dose.
• Conduct weekly on-site facility visits.
• Conduct monthly unit reviews of the department.
• Ensure stock sufficiency and coordinate with wholesalers for alternatives.
• Review controlled substances logs and conduct annual inventory.
• Provide drug information to clinical staff as needed.
• Counsel patients on therapy when needed.
• Plan, monitor and evaluate drug programs and regimens.

Pharmacist
Anchorage, Alaska
Pay From: $75.00 per hour

MUST:

• Experienced Pharmacist
• Must have an active Alaska Pharmacist license
• Must have PIC (Pharmacist in Charge) experience
• Behavioral Health is highly preferred

DUTIES:

• Pharmacist will be responsible for, but not limited to the following:
• Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose
• Conduct weekly on-site visits to each facility
• Conduct monthly unit review of department
• Assist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessary
• Review and reconcile controlled substances logs and conduct an annual controlled substance inventory
• Assist clinical staff with drug related information
• Counsel patients on medication therapy, if needed
• Plan, monitor and evaluate drug programs and regimens
