Qualifications

• Valid pharmacist license with current registration

Responsibilities

• Dispense medications accurately in accordance with regulations

Job Overview

We are seeking a dedicated and highly skilled Pharmacist to join our dynamic healthcare team. In this vital role, you will be responsible for providing expert pharmaceutical care, ensuring medication safety, and supporting patient health outcomes across various hospital settings. Your expertise in clinical pharmacy, medication management, and regulatory compliance will be essential in delivering exceptional patient service. Responsibilities

• Dispense medications accurately while adhering to pharmaceutical regulations and hospital policies
• Conduct comprehensive patient assessments to determine appropriate medication therapies and dosages
• Collaborate with healthcare providers to optimize medication regimens, including acute pain management and oncology treatments
• Prepare and compound sterile and non-sterile IV infusions using aseptic techniques in compliance with safety standards
• Monitor patient responses to medications, adjusting therapy as needed based on pharmacotherapeutics principles and physiology knowledge
• Maintain detailed documentation within Electronic Medical Records (EMR/EHR) systems and ensure proper drug regulation compliance
• Precept pharmacy residents and students, fostering clinical development and best practices in pharmacy care

Requirements

• Valid pharmacist license with current registration in the relevant jurisdiction
• Extensive hospital pharmacy experience, including hospital medicine, ICU, PICU, emergency room, or Level I/II trauma center settings
• Strong background in clinical pharmacy practice with experience in managed care, pharmaceutical regulatory compliance, and drug regulation
• Proven expertise in pharmacology, physiology, anatomy, and medical terminology relevant to acute care environments
• Hands-on experience with pharmacy information systems (PIS) and EMR/EHR platforms for medication management and documentation
• Demonstrated ability to perform patient care activities such as medication administration, patient assessment, and post-residency practice experience
• Knowledge of aseptic technique, IV compounding, drug regulation standards, and oncology or pain management is highly desirable
• Excellent communication skills with a focus on patient service and interdisciplinary collaboration
