The Children's Medical Services program is seeking a highly qualified candidate to fill the full-time, salaried position of PH RN Case Manager, based in Waycross, GA.

Under general supervision, following established policies, procedures, and protocols to provide comprehensive support to families of children with special healthcare needs. This role involves developing and managing individualized care plans, coordinating ongoing care, and facilitating transition planning starting at age 12. Will actively engage and retain families in the program while promoting CMS services throughout the district. Key responsibilities include building and maintaining relationships with community partners and participating in outreach activities. Additionally, the role also involves managing enrollments, conducting six-month and annual reviews, and ensuring continuity of care, reviewing medical records, approving billing and documentation for other nurses, and assisting with phone inquiries as needed. Success in this role requires strong organizational skills, effective communication, and a commitment to improving outcomes for families with special healthcare needs.

Preferred Qualifications:

Associate degree in nursing from an accredited college or university and completion of a PHN course within one (1) year of hire and current Georgia Registered Professional Nurse license and CPR certification.

Experience working with infants or young children. A bachelor’s degree in nursing (BSN). One year minimum experience as a registered professional nurse, and bilingual proficiency in Spanish.
