A healthcare facility in Independence seeks a Personal Support Worker to provide essential personal care and support to patients with diverse medical needs. You will collaborate with nursing staff and the interdisciplinary team to enhance patient wellness while adhering to care protocols. UNIT PROFILE
The Alternate Level of Care (ALC) unit is committed to providing high quality care to a diverse spectrum of patients with simple to complex medical issues. Patients are transferred to the ALC unit once they have been deemed medically stable and are awaiting transfer to alternative facilities or programs (ie. Long-Term Care Homes or Rehabilitation). The main focus of the ALC unit and our interdisciplinary team is special assessment, treatment and rehabilitation of elderly patients with special needs in order to achieve optimal wellness before they are discharged or while awaiting alternate levels of care in the community.
POSITION SUMMARY
Under the supervision of registered nursing staff, the Personal Support Worker will be integrated in the unit’s model of care to provide direct quality personal care to patients.
QUALIFICATIONS
• Minimum Grade 12 Education and successful completion of a 1 year diploma program as a Certified Personal Support Worker
• Current Basic Cardiac Life Support certification
• GPA and CPI certification preferred
• Previous PSW/Customer Service experience preferred
• Demonstrated verbal and written communication skills
• Record of good attendance
RESPONSIBILITIES
• Assisting patients to meet their basic physical, social, psychosocial and emotional needs in accordance with their plan of care
• Provides personal care to patients including assistance with activities of daily living, personal hygiene, nutritional needs, toileting, lifts and transfers utilizing a respectful and caring manner/demeanor
• Maintains documentation of patients’ activities of daily living on flowsheets (Dovetale) and reports to registered staff and changes in patient condition
• Engages with patient/client that aligns with a patient family centered care philosophy
• Works collaboratively with the patient, families and the interdisciplinary health care team
• Maintains a positive attitude and supports unit initiatives and change
• Participates in Transfer of Accountability at change of shift
• Adhere to SJHH policies and procedures, including IPAC policies and procedures, and code response policies, specifically utilization of prevention and intervention strategies for patients with responsive behaviours
WAGE RATE
Hourly: $28.81....$29.97....$31.11 (plus 14% in lieu of benefits)
