Responsibilities

• Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting, ensuring their comfort and dignity.
• Monitor patient health by observing vital signs, administering medications as prescribed, and maintaining accurate care plans in compliance with HIPAA regulations.
• Support residents during transfers using equipment like Hoyer lifts and patient transfer methods to promote safety for both clients and staff.
• Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption to encourage proper nutrition.
• Engage in patient observation to identify changes in condition or behavior, especially for those with dementia or autism spectrum disorders.
• Maintain a clean and safe environment through regular cleaning routines and manual handling procedures aligned with health standards.
• Provide transportation assistance for appointments or community outings while ensuring client safety during travel.
• Document all care activities clearly and accurately, communicating effectively with team members about each resident’s status and progress.

Qualifications

• Proven experience working in group homes, assisted living facilities, nursing homes, or home health environments is highly preferred.
• Knowledge of patient care techniques including medication administration, vital signs monitoring, and patient personal care assistance.
• Familiarity with specialized care such as Alzheimer’s care, memory care, dementia support, and hospice care is advantageous.
• Ability to perform manual handling tasks safely using equipment like Hoyer lifts; experience working with heavy lifting is a plus.
• Strong understanding of resident’s rights and confidentiality standards under HIPAA regulations.
• Excellent communication skills for engaging with clients effectively and documenting care accurately.
• Certification in CPR and First Aid is required; additional certifications related to healthcare or caregiving are beneficial.
• Experience working with individuals with disabilities or neurodevelopmental disorders enhances your ability to provide personalized support.
