IHA at Broad Street PACE is seeking a Personal Care Worker (CNA) to support elderly participants in daily living activities and help maintain independence within a coordinated care model.

This full-time, first shift role is based at the Broad Street facility with occasional home care. The position offers competitive pay, 1st Day Benefit coverage, Daily Pay, and opportunities for professional development.

Qualifications

• Active CNA certification in the state where PACE Organization is located.
• Minimum of one year of documented experience working with a frail or elderly population preferred.
• Valid driver's license.
• Superior written and verbal communication skills.
• Customer focused with ability to exercise independent judgment and adapt to change.

Responsibilities

• Provide personal care assistance to maintain independence in activities of daily living (toileting, bathing, eating, grooming).
• Assist participants with physical activity, exercise, and walking.
• Provide behavioral supervision or support (reality orientation, reminders for nursing and medication instruction, diet restrictions, and emotional reassurance).
• Assist with meals, recreation, and housekeeping.

Personal Care worker

Status: Full time

Location: Broad Street PACE

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position will be housed primarily at the Broad Street facility. However, occasional home care work may be required.

What You Will Do

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in the maintenance of physical activity, exercise, and walking
• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What You Will Need

• Must be an active certified nurse assistant in the state where PACE Organization is located.
• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred
• Valid driver's license
• Superior written/verbal communication skills
• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change
• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems
• Position may require 10% local travel and use of personal vehicle.
• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
