Job summary

Trinity Health in Philadelphia is seeking a dedicated Personal Care Worker to join the Broad Street PACE program. The role primarily serves the elderly through a coordinated care model, providing daily living assistance and compassionate support.

This full-time placement may involve occasional home care; the environment emphasizes patient-centered care, professional development, and use of modern technology.

Qualifications

• Must be an active CNA in the state where PACE Organization is located.
• Minimum of 1 year of documented experience working with a frail or elderly population preferred.
• Valid driver's license.
• Superior written and verbal communication skills.
• Customer focused with ability to adapt to change.
• Proficiency with MS Office and basic email/calendaring tools.
• Position may require 10% local travel and ability to lift 20 pounds.

Responsibilities

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming
• Assist participants in maintaining physical activity, exercise, and walking
• Provide behavioral supervision or support, such as reminders to follow nursing and medication instructions, diet restrictions, and emotional reassurance
• Assist with general activities such as conversing with participants, recreation, meals, and housekeeping

Location: Broad Street PACE

Our facility ministers exclusively to the elderly through a coordinated care model which provides preventive, primary, acute and long-term care services designed to assist older individuals to live as independently as possible while continuing to live within their community.

This position will be housed primarily at the Broad Street facility. However, occasional home care work may be required.

What you will do:

• Provide personal care assistance maintaining the independence of the participant to conduct activities of daily living, including toileting, showers, eating, and grooming

• Assist participants in the maintenance of physical activity, exercise, and walking

• Provide behavioral supervision or support for participants, such as reality orientation, reminder to follow through with nursing and medication instruction, diet restrictions, and emotional reassurance

• Assist with general activities such as conversing with participants, recreation activities, meal and snack serving, and housekeeping.

What you will need:

• Must be an active certified nurse assistant in the state where PACE Organization is located.

• Minimum of one (1) year of documented experience working with a frail or elderly population strongly preferred

• Valid driver's license

• Superior written/verbal communication skills

• Must be customer focused, exercise independent judgment and flexibility to adapt to ongoing change

• Proficiency with Microsoft Office (MS Word, Excel, Power Point, etc.) and basic knowledge/experience with electronic mail and calendaring systems

• Position may require 10% local travel and use of personal vehicle.

• Position requires ability to engage in frequent kneeling, crouching, reaching, lifting, sitting, walking, bending and lifting. Must be able to push, pull, move and or lift a minimum of 20 pounds if applicable.
