Job summary

Concordia Lutheran Ministries in Monroeville, PA is seeking a Weekend LPN Supervisor to ensure high-quality resident care, regulatory compliance, and outstanding service standards on the assigned unit.

This role requires a valid Pennsylvania LPN license, excellent verbal and written communication, and weekend staffing oversight. You will participate in care planning, complete required DHS assessments, and monitor care trends to support residents and staff.

Qualifications

• Licensed Practical Nurse with valid current Pennsylvania license.
• Excellent communication skills (verbal and written).
• Supervisory experience a plus.
• Knowledgeable of Department of Health Services personal care home regulations.
• Experience preferred.

Responsibilities

• Daily reports and follows up to ensure resident care issues are addressed.
• Reviews the doctor list daily and follows up to address resident care issues.
• Wellness Coordinator participates in care planning meetings weekly.
• Completes initial and annual PCH Assessments using the DHS forms timely.
• Coordinates and documents changes to the PCH Assessment on medical evaluation and support plan.
• Monitors resident care outcome trends and unit issues.
• Handles weekend staffing and covers weekend call-offs.

Education

Licensed Practical Nurse (LPN) license - PA

Excellent communication

Position Summary

The Weekend LPN Supervisor is responsible to assure the provision of high-quality care, compliance, and customer service standards on his/her assigned unit(s) making sure that the medical, physical, social, and emotional needs and general welfare of residents are met.

Essential Duties

• Daily reports and follows up to make sure resident care issues are addressed
• Reviews the doctor list daily and follows up to make sure resident care issues are addressed
• Wellness Coordinator participates in care planning meetings weekly
• Completes initial and annual PCH Assessments using the correct DHS-required form in a timely manner
• Makes sure changes and updates to the PCH Assessment are coordinated and documented on the medical evaluation and support plan
• Is knowledgeable of resident care outcome trends and issues on the unit
• Handle weekend staffing
• Occasionally cover weekend call offs
