VNA Alliance is seeking an experienced Licensed Practical Nurse to perform in-home visits in the Meadville, PA area. The role requires PA licensure, strong clinical skills, and the ability to navigate an Electronic Health Record system.

Responsibilities include documenting care, supervising team members, coordinating with physicians and other disciplines, and planning for discharge while ensuring high-quality patient outcomes. Travel to patient homes is required with reliable transportation.

Qualifications

  • Licensed Practical Nurse with a current Pennsylvania license.
  • OASIS Certified preferred.
  • Two years of progressive home health care experience preferred.
  • Strong interpersonal, communication, and organizational skills.
  • Proficient with knowledge of an electronic health record (EHR).
  • Knowledge of agency policies, federal and state regulations, and third-party payers.
  • Reliable transportation with valid driver's license and insurance.

  • Evaluates patient condition for additional services, equipment, and consultations to improve outcomes.
  • Educates team members and facilitates interdisciplinary communication.
  • Collaborates with primary clinician to meet patient needs within resource constraints.
  • Plans discharge from service at admission in coordination with clinicians.

Job description

QUALIFICATIONS
  • Licensed Practical Nurse with a current, valid Pennsylvania license.
  • OASIS Certified preferred
  • Two [2] years of progressive experience in a home health care setting preferred.
  • Interpersonal, communication and organizational skills.
  • Proficient with knowledge of an electronic health record.
  • Knowledge of agency policies, federal and state regulations, and third-party payers
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state
RESPONSIBILITIES
  1. Reviews clinical documentation for compliance and completeness in a timely manner.
  1. Oversees and offers supervision to members of their team to help assure that quality of care is provided.
  2. Evaluation of patient's condition for the need of other services, equipment, and consultations that may facilitate better overall outcomes.
  3. Assists in meetings to provide education to team members and facilitate interdisciplinary communication.
  4. In collaboration with the primary clinician, provides what is necessary for the patient's need to obtain positive medical outcomes while managing appropriate resources.
  5. Plans with the primary clinician and other disciplines for the discharge from service at the time of admission.
  6. Knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient's benefit.
  7. Liaison between patient/family, physician, and all other healthcare providers.
  8. Provides in-home nursing visits on an as-needed basis.
  9. Participates in administrative on-call rotation.
  10. Responsible for understanding patient outcomes and HHCAHPS requirements and assisting with the education of the staff as it relates to these regulatory requirements.
  11. Reviews and approves the referral for accuracy and completeness in the electronic medical record. Ensures the communication note is thorough and complete.
  12. Additional duties as assigned
DOCUMENTATION
  1. Exhibits a thorough knowledge of the agency's electronic health record documentation system.
  2. Demonstrates expertise in all aspects of required documentation completion within scope of the employee's licensure.
