Job summary

Landis Homes Personal Care seeks a Clinical Coordinator (LPN) to partner with the Director of Personal Care and shift leaders for 24/7 resident care management. You will round daily, provide clinical support, monitor incidents and infection control, and assist with direct care as needed.

The role requires PA-licensed LPN credentials and current CPR, with on-call duties and coordination with billing and scheduling. This is an on-site position in Lititz, PA within Landis Homes.

Qualifications

• Nursing/healthcare experience required.
• Licensed in PA as LPN.
• Current CPR certification required.

Responsibilities

• Work with the Director of Personal Care and nursing leadership to ensure 24-hour management.
• Round daily on each house to verify level of care needs are met.
• Provide clinical support to care teams and oversee quality issues and incidents.
• Coordinate with infection control on issues and review reports.
• Assist with direct care when needed (bathing, dining room, call bells, activities).
• On-call availability in emergencies and share on-call with the Director of Personal Care.
• Assist with staffing coordination to ensure adequate coverage per regulations.
• Coordinate monthly billing assessments with Personal Care households and submit to Finance.

Personal Care Clinical Coordinator at Landis Homes Personal CarePosition Essential Duties/Responsibilities:Works closely with the Director of Personal Care, day shift charge nurses and alternate shift RN Campus Supervisors for effective 24-hour management.Recognizes and responds to significant changes in resident condition and provides leadership in any emergency.Rounds daily on each house, ensuring level of care needs are met.Provides clinical support to the team members.Oversees quality issues, monitors incidents/accidents, provides follow-up monitoring, and addresses issues immediately.Works with RH Infection Control Preventionist in monitoring infection control issues in assigned house(s), completes forms and reviews reports.Assists with direct care, such as bathing, assists in the dining room, responds to call bells and participates in resident activities as able.Serves as a change agent, communicating change positively to staff.Is available (on-call) in an emergency situation when required staffing is affected.Shares weekly on-call with the Director of Personal Care.Works 2 shifts per month on the floor as a nurse.Works with scheduler on staffing issues to ensure that call-offs and vacancies are filled to provide adequate coverage as per regulatory requirements and resident requirements.Coordinates monthly billing assessments with Personal Care households,and submits billing to the Finance department.LPN QualificationsNursing/healthcare experience requiredLicensed in the state of PACurrent CPR certification
