Franciscan Health's Dyer Health Center South in Dyer, IN is seeking a PACE Personal Care Attendant II to provide compassionate support to seniors, coordinating care across home, day center, clinic, and transport settings.

This role emphasizes teamwork, patient comfort, and quality of life, with hours Monday–Friday 8:00 a.m.–5:00 p.m. Travel up to 75% is required to serve participants in multiple environments.

Qualifications

• High School Diploma or GED required.
• At least 1 year experience with frail/elderly populations.
• At least 1 year healthcare experience as a patient care technician preferred.
• BLS certification from American Heart Association required.
• Indiana CNA license required or HUC qualification.
• Franciscan Patient Care Assistant Program (FPCAP) required.
• Willingness to travel up to 75%.

Responsibilities

• Respond to participant needs and direct inquiries.
• Maintain inventory and care environment; assist PACE participants.
• Deliver care in home, day center, clinic, or transportation.
• Must be over 23 years old for insurability.

WHAT YOU CAN EXPECT

• Practice Hours: Monday – Friday, 8:00 a.m. – 5:00 p.m.
• No Weekends, Evenings, or Holidays
• Respond to participant needs and direct the inquiry to the appropriate party.
• Maintain inventory supplies, set-up of the care environment, and physically assist PACE participants.
• Deliver care in multiple settings, including in the home, the day center, the clinic or in transportation.
• For insurability requirements, all candidates must be over the age of 23.
  

QUALIFICATIONS

• High School Diploma/GED- Required
• 1 Year- Prior experience working with frail/elderly population- Required
• 1 Year- Healthcare experience as a patient care technician or similar position- Preferred
• Basic Life Support Program (BLS) - American Heart Association- Required
• Certified Nursing Assistant (CNA)- State of Indiana Licensing Board - Required_ OR _
• Health Unit Coordinator (HUC) - Required_ OR _
• Franciscan Patient Care Assistant Program (FPCAP) - Require
