Job summary

Mustardseed Health Care Services LLC in Frederick, MD is looking for a dedicated Home Health Aide to assist patients at home under the supervision of a Registered Nurse. You will perform basic health care tasks and personal care to promote comfort and safety.

Applicants should have at least 6 months of home health care experience, a Maryland driver’s license with insurance, CPR/First Aid cards, and the ability to read, write, and follow directions.

Qualifications

• Experienced in home health care or related health care field.
• High School graduate or equivalent.
• Demonstrates ability to read, write, and carry out directions.
• Possesses maturity and ability to deal effectively with job demands.
• Good verbal and written communication skills.
• Minimum of 6 months of experience in Home health care.
• Possesses Maryland driver's license and automobile insurance.
• Current CPR card/First Aid card
• Health clearance/physical within 12 months.
• Criminal background clearance.
• Reliable transportation.
• I-9 Documentation.
• Social Security.
• W-4 Federal Income Tax forms.
• State Tax forms.
• TB or Chest X-ray within 12 months.
• Hepatitis B series/declination.
• At least Two (2) letters of Reference (must be in good standing).

Responsibilities

• Performs simple procedures as an extension of health care services under the direction and supervision of a Registered Nurse.
• Perform personal care activities, including but not limited to:
• Bathing
• Shampooing
• Skincare/nail care
• Oral Hygiene
• Shaving
• Dressing
• Meal prep
• Light housekeeping
• Laundry
• To ensure the quality and safe delivery of health care services.
• Follows patient plan of care as written and discussed with the Registered Nurse.
• Documentation to reflect the delivery of quality and safe health care services in the home.
• Identifies and report needs and changes in patient's condition or treatment.
• Accurately reports to the Registered Nurse their findings and/or results in a timely manner.
• Demonstrates the safe use of equipments and supplies effectively and efficiently.
• Promotes personal safety and a safe environment for the client by observing infection control practices and reporting unsafe situations to the Registered Nurse.
• Participates in patient case conferences according to Agency policy and procedure.
• Participates in education programs.
• Complies with Agency Policy and Procedure.
• Must complete at least 12 in-service courses within 12 months
