POSITION SUMMARY

The Registered Nurse (RN) ensures quality and safe delivery of services and coordinates conditions of these services that reflect the Agency’s philosophy and the standards of health and nursing care of assigned clients.

QUALIFICATIONS
  1. Graduate of a program accredited school of nursing required.

  2. Legally-Currently licensed by the State Board of Nursing to practice as a Registered Nurse in the State of Maryland.

  3. Experienced as an RN with one of the following:

    1. 1 year of acute care experience.

    2. 1 year of intermittent home health care experience.

    3. 1 year of experience in community health or public health nursing.

    4. 1 year of experience in pediatric care.

  4. Maryland driver’s license and automobile insurance.

  5. CPR card.

  6. Health clearance/Physical within 12 months.

  7. Criminal background clearance.

  8. Reliable transportation.

  9. Good written and verbal communication skills.

  10. I-9 Documentation.

  11. Social Security.

  12. W-4 Federal Income Tax Forms.

  13. State Tax Forms.

  14. TB or Chest X-Ray within 12 months.

  15. Hepatitis B series/declination.

  16. At least Two (2) letters of reference (must be in good standing).

DUTIES
  • Make initial evaluation visit on all assigned patients within 24 hours of the first contact with the patient/family in order to:

  • Determine whether the agency has the ability to adequately provide the necessary services in the home.

  • Collect comprehensive data from the client/family or significant others.

  • Initiate or complete the plan of treatment.

  • Communicate the evaluation of the patient’s condition to the Physician.

  • Re-evaluate the patient’s condition and inform the Physician and other members of the multidisciplinary team of changes.

  • Revise the plan of care as indicated by the patient’s condition and nursing needs.

  • Request modification of orders based on nursing judgment and regulatory requirements.

  • Initiate appropriate preventive and rehabilitative nursing measurements.

  • Counsel the patient and family in meeting nursing and related needs of the patient.

  • Prepare clinical and progress record documentation reflecting delivery of quality and safe home care per Agency policy services.

  • Supervisory visits of Certified Nursing Assistant, Certified Medicine Aide and Geriatric Nursing Assistant every 30 days.

  • Supervises and Directs the LPN every 2 weeks in the implementation of the plan of care.

  • Implements Nursing interventions to meet client’s needs and changed conditions.

  • Document for Client’s clinical record all care provided per Agency policy.

  • Monitors Client care for compliance with defined client plans of care.

  • Fulfills additional performance responsibilities as assigned and as necessary.

  • Participation in monthly health care staff meetings.

  • Participates in appropriate committee meetings.

  • Supervises Licensed Practical Nurses and Certified/Geriatric Nursing Assistants.

  • Participates in orientation of health care staff members regarding equipment and supplies accurately and in a cost effective manner.

  • Participation in developing systems or re-evaluating health care agency delivery of services.

  • Participation in meeting requirements for state and health department surveys for the agency.

  • Ensure that regulatory documentation forms are completed and submitted in a timely manner.
