Job summary

Baptist Life Communities is seeking a Personal Care Assistant to support adults 55+ in maintaining an active, healthy lifestyle. The role emphasizes person-centered care, teamwork, and adherence to facility policies while assisting with daily living tasks defined by each resident's care plan.

You will participate in QAPI initiatives, attend trainings, and collaborate with the nurse manager to ensure quality of life and safety for residents, with a focus on compassionate, respectful care delivery.

Responsibilities

• Learn and perform PCA essential duties under supervision of the Assisted Living Nurse Manager.
• Attend trainings for QAPI understanding and participation.
• Carry out QAPI roles and responsibilities as assigned.
• Follow established facility policies and procedures to support QAPI initiatives.
• Support QAPI efforts through actions and attitude, adjusting practices accordingly.
• Model outstanding customer service with a person-centered care focus.
• Deliver direct personal care per care plans and training standards.
• Keep resident rooms tidy and respect residents' rights and preferences.
• Assist with meals, nourishment, and meal tray collection.
• Transport residents to dining, activities, therapy, or beauty salon as needed.
• Provide ice, water, and other refreshments as assigned.
• Complete tasks on the unit's assignment sheet for the shift.
• Report incidents, symptoms, or observations to the Nurse Manager.
• Report any abuse or neglect per facility policy.
• Attend staff meetings as directed.

Nurse Manager

EDUCATION, QUALIFICATIONS AND CREDENTIALS

High School diploma / GED required. Demonstrates knowledge of and ability to apply principles of customer service to a high degree. Demonstrates the ability to work effectively with all levels of personnel. Demonstrates the ability to utilize appropriate judgement in requesting assistance from licensed personnel. Ability to communicate well through speech, written word and body language to denote approval or displeasure. Ability to summarize and synthesize information and communicate the message to others. Ability to foster teamwork.

JOB ROLE

• Learn and become proficient in the essential job functions of a PCA under the direction and supervision of the Assisted Living Nurse Manager.
• Attend trainings to build understanding and capacity to undertake QAPI work.
• Carry out QAPI roles and responsibilities as assigned.
• Follow established policies and procedures in support of QAPI efforts.
• Support QAPI efforts both verbally and non-verbally (i.e., via actions and attitude), including adjusting performance and practice in accordance with QAPI initiatives and findings.

ESSENTIAL JOB FUNCTIONS

• Consistently model and demonstrate excellence in customer service, with a constant focus on person-centered care/services delivery.
• Delivers direct personal care tasks as described on each resident's care plan and as directed per training standards.
• Maintains the residents' rooms in a tidy, orderly condition with respect to residents' individual rights and preferences.
• Serves residents' meals and nourishments and collects meal trays when meal is completed.
• Transports residents to the dining rooms, activities, therapy department and/or beauty salon as needed per their request and/or schedule.
• Supply fresh ice, water and other refreshments as assigned.
• Completes all tasks as directed on the unit's assignment sheet for the assigned shift.
• Reports any unusual occurrences (incidents), signs, symptoms, complaints or observations of the resident to the Nurse Manager.
• Reports any allegation of abuse, neglect, exploitation or misappropriation of resident property per facility policy.
• Follows the policies and procedures of the facility.
• Completes annual continuing education requirement on an annual basis.
• Attends staff meetings as directed.

KNOWLEDGE, ABILITIES AND SKILLS

Knowledge of current federal and state laws and regulations that apply to Assisted Living setting. Has strong organizational and time management skills and the ability to prioritize responsibilities. Ability to identify problems, collect data, establish facts and draw valid conclusions.
