Job Title: Personal Care Aide
Qualifications:
- Must be at least 18 years of age (21 years old with 1 year experience and good communication skills preferred)
- Minimum of a High School Diploma or GED
- Submit to and pass a criminal record check and a drug screen test
- Possess a valid social security card or valid work permit for non-US citizens
- Possess a valid driver’s license with no moving violations (must be 25 years old to drive an agency vehicle)
Hours:
Monday through Friday 8:00 am until 4:30 pm. Evening and weekend hours may be required.
Reporting:
This position reports directly to the Team Leader or Program Director.
Summary of Job Duties:
The Personal Care Aide is a part-time position paid hourly. The role involves facilitating independence and promoting community integration for individuals with mental illness.
Essential Duties and Responsibilities:
- Provide therapeutic training to promote independence according to Individual Plan of Care goals
- Assist individuals to be as independent as possible
- Support personal care needs and household chores
- Assist with sanitation, laundry, meal prep, and planning
- Engage in community activities and relationship building
- Ensure safety and report issues in the home environment
- Provide financial and money skills training
- Organize fun activities and leisure options
- Assist with shopping and connecting individuals with family and friends
- Support behavioral intervention plans and record data
- Monitor health, promote exercise, and advocate for rights and services
- Respect privacy and maintain confidentiality
- Accurately document incidents, medication administration, and daily notes
- Communicate effectively and demonstrate initiative
- Be dependable, punctual, and professional in appearance
- Collaborate with team members and follow instructions
- Perform other duties as assigned

Additional Responsibilities:
- Physical ability to perform job tasks (reach, push, pull, lift up to 20 pounds)
- Maintain a valid driver’s license and safe driving record
- Complete TB test and training requirements
Work Environment:
- May work outdoors and off-site at residential programs
- Noise level is mild to moderate
Environmental Data and Job Hazards:
- Irregular hours and potential exposure to infectious materials
- Participation in annual infection control and safety training
