Job summary

Community Horizons supports a Tomah, WI community member who is ambulatory with walkers and uses a wheelchair. The caregiver will provide standby assistance and everyday home care, including ADL assistance, support with walking, errands, light cooking and cleaning & organizing.

The client is independent with showers but may want assistance; shower chair, raised toilet seat and grab bars are used. A valid driver’s license is preferred.

Member is ambulatory with walkers, working on walking more, and uses a wheelchair. Member can be independent with showers but might want assistance. They use a shower chair, raised toilet seat and grab bars to aid. This member is looking for help with stand by assistance and also everyday home cares.
