Devon County Council is seeking a dedicated Social Care Assessor to join our Adults Social Care Team. The role involves conducting strengths-based, person-centred assessments and developing and reviewing support plans in line with the Care Act 2014.

You will help individuals access services and work with families, carers, health professionals and partner organisations to promote safeguarding and wellbeing across Devon.

Competenze

• Experience in health or social care settings.
• Knowledge of person-centered approaches and relevant social care legislation.
• Excellent assessment and interpersonal skills.

Mansioni

• Carry out strengths-based, person-centred assessments of care and support needs.
• Develop and review support plans in line with Care Act 2014.
• Support individuals to access services, community resources and specialist support.
• Work collaboratively with families, carers, health professionals and partners.
