Aspire seeks a Work Life Consultant (Case Manager) to partner with individuals with disabilities, their families, and teams to honor goals through person-centered planning and support. You will advocate, coordinate services, and coach DSPs to ensure dignity, independence, and quality outcomes.

Qualifications

• Bachelor’s degree in a human services-related field is required.
• Minimum of one year experience supporting individuals with disabilities.
• Illinois QIDP Certification or eligibility.
• Valid driver’s license with a good driving record.
• Working knowledge of standard software applications.

Responsibilities

• Develop and implement individualized service strategies aligned with each person’s Personal Plan.
• Monitor progress and ensure quality and compliance with DHS standards.
• Coach DSPs in implementing individualized plans effectively.
• Visit individuals during Academy hours to monitor progress and provide direct support.
• Coordinate meetings with providers to optimize outcomes and service delivery.
