UCP Seguin of Greater Chicago is seeking a Community Living Coordinator to monitor and oversee Intermittent CILA services, home and community based respite, and semi-independent living settings, ensuring residents develop daily living and interpersonal skills in line with DHS licensing specifications.
The role emphasizes safety, compliance with IFP goals, and growth in participants' independence, with a Monday–Friday schedule and after-hours pager responsibilities as needed to support program
To provide effective monitoring and oversight of Intermittent CILA Services, Home and Community Based respite services, and Semi-independent Living homes, in a manner which ensures program activities and environment are consistent with IFP goals and objectives, promotes the health and safety and growth in residents' acquisition of daily living and interpersonal skills needed to function as independently as possible in the community in accordance with applicable certification and DHS licensing requirements and standards.
Selects and supervises (orients, trains, schedules, evaluates, disciplines, commends, and directs daily activities) assigned subordinate staff utilizing measurable performance evaluation criteria for staff growth and ensuring performance standards necessary to meet the needs of persons served are met in a manner consistent with the mission and objectives of the Agency and the services it provides.
Carry and respond to Agency pager within departmental designated timeframes 24 hours per day 7 days per week, except when on General Leave or when other arrangements are made with supervisor.
Monday through Friday, regular business/program hours with scheduling flexibility to work variable hours as needed to appropriately carry out duties and responsibilities effectively.
