Job summary

Family Promise Big Bend is seeking a Permanent Supportive Housing & Care Management Coordinator to oversee housing stabilization and care coordination for individuals experiencing homelessness. The role includes direct advocacy, multidisciplinary collaboration, and ensuring program compliance.

You will supervise case managers, maintain essential records, and engage with community partners to connect residents with housing resources.

Qualifications

• Bachelor’s degree in Business Administration, Psychology, Social Work, Public Health or related field preferred.
• 2–3 years’ experience with vulnerable populations facing homelessness, crises, trauma or mental health/substance use issues.
• Strong knowledge of HUD, Section 8 regulations and fair housing practices.
• Excellent communication and interpersonal skills.
• Proficiency in Office Suite (Word, Excel, PowerPoint, Outlook) required.
• Organizational and problem-solving skills; attention to detail; multitasking ability.

Permanent Supportive Housing & Care Management Coordinator

Position Type: Full-Time

POSITION SUMMARY

The Permanent Supportive Housing (PSH) & Care Management Coordinator oversees daily coordination of housing stabilization and care management services for individuals experiencing homelessness. This role combines direct care advocacy, multi-disciplinary care coordination and programmatic compliance. This role requires strong organizational, communication, and problem-solving skills to maintain a high standard of living for residents. This position will oversee case managers in residents. The Coordinator will report to the Director of Permanent Supportive Housing & Care Management.

MINIMUM REQUIRED QUALIFICATIONS

• Bachelor’s degree in Business Administration, Psychology, Social Work, Public Health or a related field is preferred.
• Preferred 2-3 years’ experience working directly with vulnerable populations experiencing homelessness, crisis, complex trauma, severe mental illness or substance use disorders.
• Strong knowledge of HUD, Section 8 regulations and fair housing practices.
• Excellent communication and interpersonal skills.
• Proficiency in Office Suite (Word, Excel, PowerPoint, Outlook) is required.
• Organizational and problem-solving skills.
• Attention to detail and the ability to multitask.
• Experience working directly with diverse homeless, underserved populations/families in a fast-paced environment.
• Knowledge of community resources and social service systems.
• Experience with crisis intervention and de-escalation.

REQUIRED SKILLS AND ABILITIES

• Ability to communicate effectively in writing and verbally while providing exceptional customer service.
• Excellent organization and time-management skills with the ability to manage multiple priorities and manage a team of case managers.
• Strong documentation and recordkeeping skills using Microsoft Office, HMIS and other databases.
• Ability to conduct effective care management sessions and housing-focused assessments.
• Ability to work independently while functioning effectively as part of a multidisciplinary team.
• Ability to exercise sound judgment, competence, flexibility, sensitivity, and always maintain professionalism.

ADDITIONAL REQUIREMENTS

• Valid Florida driver's license and reliable transportation.
• Ability to travel locally for community meetings, appointments, and other program-related activities.
• Flexibility to participate in occasional evening or weekend activities as required.
• First Aid and CPR certification within 30 days of employment, if not currently certified.

JOB TYPE

Full time (position will require local travel and occasional work hours outside of regular M-F work week).

PAY RANGE

$45,900 Annually
