Responsibilities

  • Conduct comprehensive speech, language, cognitive, and swallowing evaluations.
  • Develop individualized treatment plans based on each resident's needs and goals.
  • Provide skilled speech-language therapy using evidence-based clinical practices.
  • Monitor patient progress and modify treatment plans as appropriate.
  • Evaluate and treat communication, cognitive, and swallowing disorders.
  • Complete accurate and timely clinical documentation.
  • Collaborate with nursing, therapy, dietary, and other healthcare professionals.
  • Participate in interdisciplinary care planning.
  • Educate patients, families, and caregivers regarding treatment strategies and recommendations.
  • Communicate effectively with the care team regarding patient progress and changes in condition.
  • Promote patient safety, independence, communication, and quality of life.
