Monitor patient progress and adjust treatment plans as needed
Document evaluations, treatments, progress, and outcomes accurately and efficiently
Collaborate with nursing, therapy, and other interdisciplinary team members
Educate patients, families, and caregivers on therapeutic techniques and strategies
Participate in care planning and contribute to positive patient outcomes
Maintain compliance with clinical standards, facility policies, and applicable regulations
Graduate of an accredited Occupational Therapy program
Current California Occupational Therapist license or eligibility for licensure
Strong communication and interpersonal skills
Compassionate, patient-focused approach to care
Ability to work effectively as part of an interdisciplinary team
Strong organizational and clinical skills
Previous SNF experience is a plus, but not required
