We are seeking a dedicated and compassionate Occupational Therapist (OT) to join an 80-bed skilled nursing facility in beautiful Canon City, Colorado. This permanent opportunity offers a consistent Monday–Friday schedule with no weekend rotation, giving you the opportunity to provide meaningful patient care while maintaining a strong work-life balance.
You'll work closely with patients to help them regain functional independence, improve their ability to perform daily activities, and reach meaningful therapy goals. In a smaller SNF environment, you'll also have the opportunity to build strong relationships with patients and collaborate closely with the interdisciplinary care team.
$42–$47/hour compensation

Perform comprehensive occupational therapy evaluations and assessments
Develop individualized treatment plans based on patient needs and functional goals
Provide skilled OT interventions to improve patients' independence and quality of life
Address activities of daily living, functional mobility, strength, coordination, and other functional limitations
Monitor patient progress and adjust treatment plans as needed
Maintain accurate and timely clinical documentation
Collaborate with nursing, rehabilitation, and other interdisciplinary team members
Educate patients, families, and caregivers on therapeutic techniques and strategies
Participate in care planning and contribute to positive patient outcomes
Follow facility policies, clinical standards, and applicable regulatory requirements
Graduate of an accredited Occupational Therapy program
Current Colorado Occupational Therapist license or eligibility for licensure
Strong communication, organizational, and clinical skills
Compassionate and patient-centered approach to care
Ability to work effectively within an interdisciplinary team
SNF experience is preferred but not required
