Job summary

The Child Center of NY is seeking a Permanency - Family Connections Specialist, RTF to promote timely discharge planning and lasting family connections for youth in our Residential Treatment Facility. You will collaborate with families, care coordinators, and community partners to build sustainable support networks and ensure successful community integration.

Qualifications

• Master's degree in social work or related field (LMFT, LMSW, LMHC).
• Master in Social Work preferred.
• Familiarity with OCFS and OMH policies and procedures.
• Experience with adolescent youth with psychiatric disorders.
• Experience in Wraparound Services model and strengths-based approaches.

Responsibilities

• Secure additional service resources to expedite discharge and permanency.
• Assist transitional coordinator with community resources for homes.
• Provide therapeutic support sessions with youth and family post-discharge.
• Develop emotional permanency resources when reunification is not possible.
• Submit and monitor needs of RTF clients related to family connection and permanency.
• Coordinate with transitional coordinators, staff, and community providers to finalize permanency.
• Complete timely and accurate documentation.
• Supervise the Family Peer Advocate; other duties as assigned.

Education

Master's degree in social work or human services related field (LMFT, LMSW, LMHC)

Master in Social Work (Preferred)

Excellent written and oral communication skills

Experience with adolescent youth with psychiatric disorders

Wraparound Services model experience
