Job summary

The Child Center of NY in New York City seeks a Permanency - Family Connections Specialist, RTF to promote timely discharge planning and lasting family ties for youth in our Residential Treatment Facility. You will use the Family Finding model to engage families and supportive adults, coordinating with care teams, counties, and partners to build lasting networks.

Qualifications

• Master’s degree in social work or human services field with licensure options.
• Master in Social Work preferred.
• Bilingual English/Spanish preferred.
• Knowledge of OCFS and OMH policies and procedures.
• Experience with adolescent youth with psychiatric disorders.
• Wraparound Services model experience.
• Strengths‑based, person‑centered approach.
• Minimum 3 years with diverse client caseload including youth and families.
• Strong written and oral communication skills.
• Computer proficiency for documentation.
• Experience in residential or hospital settings preferred.

Responsibilities

• Works to secure additional service resources to expedite discharge permanency for RTF recipients.
• Assist transitional coordinator with community resources to prep home for the youth's family.
• Provide therapeutic support sessions with youth and family post discharge.
• Assist with emotional permanency when reunification isn’t possible.
• Submit, monitor, and follow up on the needs of RTF clients regarding family connection and permanency.
• Provide linkage support between coordinators, staff, and community providers to finalize permanency.
• Complete timely and accurate documentation.
• All other duties as assigned.
