Job summary

Merakey seeks a PRN Permanency Coordinator to support SWAN units across Clearfield County and surrounding areas, delivering Child Profile, Child Preparation, Child Specific Recruitment, Family Profile, Placement, Finalization and Post Permanency services to promote timely permanency.

The role requires excellent writing, communication, organizational and computer skills, a valid driver's license, and willingness to travel with some evening hours.

Qualifications

• High School Diploma or equivalent and at least one year of experience in child welfare or related field.
• Excellent writing, communication, organizational and computer skills.
• Travel and some evening hours; valid driver’s license and use of personal automobile required.

Responsibilities

• Deliver SWAN units of service including Child Profile, Child Preparation, Child Specific Recruitment, Family Profile, Placement, Finalization and Post Permanency Services.
• Complete units of service according to benchmarks and timeframes to promote timely permanency.
• Collaborate with stakeholders to support permanency outcomes for children.

The Permanency Coordinator works through the Statewide Adoption and Permanency Network (SWAN) to deliver SWAN units of service to including Child Profile, Child Preparation, Child Specific Recruitment, Family Profile, Placement, Finalization and Post Permanency Services (Assessment, Advocacy/Case Management, Respite, Support Group) in an effort to support permanency for children. The Permanency Coordinator will complete all assigned units of service according to established benchmarks and timeframes in order to promote timely permanency. Excellent writing, communication, organizational, and computer skills required. Travel and some evening hours necessary. Valid driver's license and use of personal automobile required.
