Job summary

Mount Prospect Academy in Pike, NH is seeking a compassionate Permanency Coordinator to support youth ages 11-21 and their families. You will collaborate with treatment teams, schools, and community partners to establish lifelong supports and stable connections that promote successful transitions back to communities.

The role involves court advocacy, family meetings, and coordination of treatment planning, with daily visits and strong emphasis on relationship-building and resource linkages.

Qualifications

  • Bachelor's degree required in social services or related field.
  • Minimum 2 years of experience working with emotionally or behaviorally challenged youth, adolescents, or families.
  • Valid driver's license and ability to pass background checks.

Responsibilities

  • Build strong relationships with youth, families, referral sources, and community partners.
  • Facilitate family engagement and strengthen support systems.
  • Coordinate family meetings, communication, and treatment planning.
  • Attend court hearings and advocate for youth treatment needs.
  • Conduct home, school, and community visits.
  • Identify and connect families with community resources and support services.
  • Provide psychoeducation and parent support.
  • Develop home safety plans and intervention strategies.
  • Collaborate with clinicians, educators, residential staff, and case managers.
  • Complete documentation, progress reports, and discharge planning.
  • Support youth and families during and after program discharge.

What You'll Do

As a Permanency Coordinator, you will:
