Qualifications

Responsibilities

• Performs the Nursing Process with evidence-based practice and individualized care.
• Delivers relationship-based patient-centered care reflecting diverse populations.
• Partners with patients and families, updating goals and care plans.
• Coordinates delivery and documentation of safe, quality patient care across the team.
• Demonstrates teamwork, participates in governance and performance improvement.
• Acts as preceptor or mentor to staff and students as needed.
• Promotes evidence-based practice and research within nursing care.
• Plans care across the care continuum with care managers and health-team colleagues.
