Job summary

Qualifications

• BSN or RN with maternity/infant/public health experience.
• BLS CPR within 90 days of hire and maintained during employment.
• Bilingual (Spanish/English) preferred.
• Experience in patient counseling and health education.

Responsibilities

• Coordinate pre and postnatal risk assessment with referrals to nutrition, social work, mental health, and medical providers.
• Educate clients on fetal development, nutrition, breastfeeding, and family planning.
• Conduct home or office visits and maintain client confidentiality.
• Communicate effectively with diverse populations; participate in monthly provider meetings.
• Attend Community First Steps meetings and support interdisciplinary care.
• Maintain documentation in MSS records and coordinate with MSS team.
