Endeavor Health is seeking a Perinatal Family Support Specialist for Evanston, IL, with occasional coverage at Highland Park Women's Hospital. The role supports high-risk pregnant and postpartum teens and adults, neonates, and families within a multidisciplinary team to improve outcomes and safe discharge.

Primary site is Evanston Hospital, with coverage at Highland Park Women's Hospital as needed. Strong emphasis on social work ethics, documentation, and collaboration across departments.

Qualifications

• Master's degree in social work, counseling, or related behavioral health field.
• Current IL license LCSW/LSW/LCPC/LPC or Licensed PsyD.
• PMH-C certification (progress toward or active).
• Two years' clinical experience; 3-5 years preferred.
• Experience with high-risk pregnant persons and families; knowledge of perinatal loss preferred.

Responsabilités

• Perform skilled psychosocial assessment and crisis intervention related to a patient's emotional and cognitive status.
• Provide social-emotional and environmental support services; refer to community resources as needed.
• Provide resources and referrals to behavioral health services.
• Perform triage duties related to patient referrals.
• Document accurately in the electronic medical record system.
• Lead in identifying barriers to service delivery and participate in improvement initiatives.
• Recognize strengths and pursue ongoing professional development.
• Adhere to NASW Code of Ethics and ongoing education requirements.

Hourly Pay Range:

$31.06 - $46.59 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

• Position: Perinatal Family Support Specialist
• Location: Evanston, IL
• Full Time/Part Time: Full time
• Hours: Monday-Friday, 8:00a.m.-4:30p.m.
• Required Travel: May be required to travel to Highland Park when needed

We are currently seeking a Perinatal Family Support Specialist; the primary site is Evanston Hospital, with some coverage at Highland Park Women's Hospital, on occasion, if needed.

Our program works closely with a multidisciplinary team supporting in and out-patient high-risk pregnant and postpartum teens and adults, neonates, pediatric patients, and their families. Additionally, we provide support around adoption and surrogacy, as well as obstetrical and gynecological surgeries; perinatal loss comprises part of the work. This position supports improved patient outcomes, safety, appropriate and timely treatment, and safe discharge.

What you will do:

• Perform skilled psychosocial assessment and/or crisis intervention related to a patient's emotional and cognitive status, current and/or historic mental health concerns, and perception of diagnosis of self and/or fetus/neonate, coping skills, support system, resource needs and barriers to care.
• Provide social-emotional and environmental support services, referring to community resources, as needed
• Provide resources and referrals to behavioral health services
• Perform triage duties related to patient referrals
• Consistently compose and enter accurate, concise, thorough and timely documentation in the electronic medical record system
• Provide leadership in identifying barriers to effective service delivery or problems in work assignments. Actively contribute to initiatives that improve service delivery through department, hospital or community based committees.
• Recognize professional strengths and areas for growth and be accountable for development and enhancement of professional practice skills.
• Adhere to NASW Code of Ethics and standards for continuing education, participation in meetings and availability for cross-coverage assignments as needed.

What you will need:

• Education: Masters level degree from accredited institution in social work, counseling, or other behavioral health science. LCSW, LCPC, Licensed PsyD; LSW (actively working towards LCSW licensure) or LPC (actively working towards LCPC licensure)
• License: Current IL license LCSW or LSW or LCPC or LPC, or Licensed PsyD required
• Certification: PMH-C (progress toward or active certification)
• Experience: Two years' clinical experience; 3-5 years, preferred
• Unique or Preferred Skills: Two or more years of experience with high-risk, at-risk pregnant persons and families, preferred. Knowledge / experience with perinatal loss
