Job summary

Rehab 2 Perform LLC is seeking a skilled Performance Physical Therapist to assess and treat patients with musculoskeletal, neurological, and movement limitations. You will develop personalized plans using evidence-based techniques, manual therapy, and functional training to help patients restore function and perform now, and for life.

You will collaborate with a multidisciplinary team, educate patients and families, and maintain thorough documentation.

Qualifications

• Graduate of a CAPTE-accredited school of Physical Therapy or a school approved by the World Congress of Physical Therapy, preferably Doctorate of Physical Therapy (DPT).
• State licensure or eligibility for state licensure.
• CPR certification at time of hire.
• SFMA Level 1 (Preferred).
• CSCS or CFSC (Preferred).
• Experience in Strength and Conditioning setting is a plus.

Responsibilities

• Evaluate and assess patients' physical condition and develop appropriate treatment plans.
• Provide hands-on therapy including exercises, stretches, and manual techniques.
• Monitor progress and adjust treatment plans as necessary.
• Educate patients and families on home exercise programs and self-care.
• Collaborate with other healthcare professionals to coordinate patient care.
• Maintain accurate documentation of evaluations, progress, and treatment plans.
