Job summary

Exos is seeking a Performance Physical Therapist in Phoenix, AZ to advance our rehabilitation system and provide high-quality care to athletes and active clients.

The role focuses on evaluating, treating, and guiding patients through evidence-based PT protocols in an outpatient orthopedic setting.

Qualifications

• Doctor of Physical Therapy degree required.
• Current CPR/BLS certification preferred.
• PT licensure in state of practice required.
• ATC/CSCS/OCS or similar certifications preferred.
• Experience with EXOS through internships or mentorships.
• ≥2 years of outpatient orthopedic PT experience.

Responsibilities

• Assist in developing the EXOS rehabilitation system in compliance with laws and licensing.
• Review physician referrals and patient records to determine PT needs.
• Perform tests, measurements and evaluations and record findings to aid in treatment planning.
• Administer appropriate physical therapy treatment and educate/motivate patients.
• Screen training athletes using FMS and EXOS UQ/LQ to determine readiness and safety.
• Develop prehabilitation protocols from screening data as needed.
• Supervise PTA and support staff in delivery of patient care.
• Document daily treatment, progress and outcomes in company records.
• Complete billing documentation in a timely manner per policy.
• Update and reevaluate treatment plans to achieve optimal outcomes.
• Collaborate with other medical services regarding patient status.

The primary duties of a Physical Therapist are: * Assist in the development of the EXOS rehabilitation system in accordance with all local, state, and national laws and regulations, as well as licensing and insurance certification* Reviews and evaluates physician's referrals and patients' records to determine physical therapy treatment requirements* Performs patient tests, measurements and evaluations and record findings to aid in developing and/or revising specifics of the treatment program or training program* Administers appropriate treatment as indicated. Instructs, motivates, and assists patients to achieve their goals* Screens incoming training athletes utilizing FMS and EXOS UQ/ LQ screening process to determine training level and safety* Utilizes screening information to develop prehabilitation protocol and interventions if needed* Appropriately supervises and directs PTA's or support staff in providing patient care and performance of their job responsibilities* Records patients' treatment, response, and progress daily within company’s documentation format* Completes documentation requirements and billing procedures in a timely manner per policy* Updates treatment plan and/ or reevaluates patients as needed to ensure optimal outcome* Interacts and consults with other medical services with regard to patients' status* Develop relationships with outside medical support staff, including but not limited to orthopedic, internal medicine, dental, chiropractic and massage* Is responsible for assisting Billing Service, as requested, to complete billing in a timely manner* Other duties as assigned**Qualifications:*** Degree in Physical Therapy* Current CPR certification (Basic Life Support (BLS) certification preferred)* PT Licensure in state of practice* ATC, CSCS, SCS, and OCS or like certifications are preferred, but, not required* Educational experience with EXOS through previous Internship or Mentorship program* 2+ years of Physical Therapy experience in an outpatient orthopedic setting**WHO WE ARE**For nearly 30 years, Exos has been dedicated to getting people ready for the moments that matter by promoting a holistic approach to health.
