Innovive Health is seeking a licensed Occupational Therapist to perform home visit evaluations, develop patient care plans, and lead therapy sessions under physician orders. You will document progress, educate patients and families, and coordinate with physicians and care teams.

The role requires a graduate OT degree, Colorado licensure, CPR certification, and 1 year of relevant experience, plus a valid driver’s license and access to a vehicle. Local travel is daily.

Qualifications

• Graduate degree in Occupational Therapy from an accredited university.
• Valid license to practice Occupational Therapy within Colorado.
• Current CPR certification.
• 1 year of home care, skilled nursing facility, acute care, or inpatient rehab experience required.
• Valid Driver's License and access to a vehicle.

Responsabilités

• Conducts home occupational therapy visits; performs evaluations, follow up visits, 30-day re-evaluations, re-certs, and discharges.
• Evaluates patient rehabilitative status and helps develop the total plan of care.
• Initiates therapy program and trains other personnel in therapy methods.
• Communicates with physicians regarding therapy frequency, duration, and goals.
• Prepares and submits clinical notes and progress summaries.
• Educates patients and families; demonstrates home exercise programs.
• Assesses vitals for safety and reports abnormal findings to supervisor.
• Educates on proper use of DME and orders DME when appropriate.
• Documents patient response using EMR; notes progress and adjusts rehab plans.
• Completes discharge planning with physicians, nurses, social workers, and others involved.
• Guides continuation of therapy after discharge; designs home exercise programs and recommends follow-up.
• Maintains confidentiality and safe practice; follows infection-control policies.
• Maintains equipment and safe work environment; follows procedures.
• Attends educational workshops; keeps up with professional publications and networks.
• Complies with federal/state regulations; assists management on required actions.
• Performs additional duties as assigned.
