Stella Maris Home Health is seeking a Registered Nurse (RN) for per-visit admissions to provide hospice start of care visits across Baltimore County/City. The role pays per visit with higher rates on weekends. Weekend visits include a $80.00 premium and start of care visits at $175, with weekend starts at $250.

Experience in home hospice is required. The position covers territories throughout Baltimore County/City on a flexible PRN schedule.

Qualifications

• Must have Home Hospice experience.
• Ability to perform hands-on skilled nursing care.
• Strong assessment and care planning skills.

Responsibilities

• Provide primary nursing care for Home Care patients in hospice start of care visits.
• Perform follow-up visits and update care plans as needed.
• Educate and supervise caregivers and volunteers in patient care.
