Qualifications

• Provide physical, psychosocial, and spiritual care to patients and families.

Responsibilities

• Provide primary nursing care under supervision to Home Care patients.
• Maintain communication to revise the plan of care and ensure quality home care.
• Teach and supervise primary caregivers and volunteers in patient care.
• Coordinate services with social workers, chaplains, therapists, and physicians.
• Order equipment, supplies, and medications as needed; document changes promptly.
• Complete documentation and charge slips accurately and timely.
