Job summary

Stella Maris Inc. is seeking a Home Hospice RN to join the growing Home Hospice team in central Maryland. The role supports patients in the community and requires coordinating admissions, educating families, and developing care plans.

Qualifications

• RN license in Maryland required.
• Prior hospice or home health experience required.
• EPIC experience preferred.
• Valid driver’s license and reliable transportation required.

Responsibilities

• Serve as main point of contact for information about home hospice services to prospective patients and families.
• Explain insurance benefits, coverage, patient responsibilities, and payment options.
• Coordinate admissions and complete paperwork promptly and accurately.
• Develop initial plan of care based on input from patient, family and physician.
• Provide information to physicians, discharge planners and community organizations about Stella Maris Home Hospice services.
• Assume on-call responsibilities as needed.
