Job summary

Stella Maris Home Health is seeking an RN for per-visit hospice start of care visits across Baltimore County/City. The role covers multiple territories with a flexible schedule and weekend rates.

Start of Care visits have higher per-visit pay and there are additional rates for specialty visits. Responsibilities include providing primary nursing care, coordinating with a care team, teaching caregivers, and ensuring documentation and regulatory compliance.

Qualifications

• Must be a licensed RN with Home Care experience.
• Ability to perform hands-on skilled nursing care.
• Experience teaching and supervising caregivers.
• Comfort with crisis intervention and patient advocacy.

Responsibilities

• Provide primary nursing care under supervision.
• Communicate to revise care plan as needed.
• Coordinate services with social workers, chaplains, therapists, dietitians and physicians.
• Document visits and mileage; complete required forms.
• Order equipment, supplies and medications as needed and ensure removal after death or discharge.
