Job summary

The Per Visit Registered Nurse (RN) at Day Kimball Health delivers skilled nursing care in patients' homes in Thompson, CT. You will act as a primary nurse on an interim basis when a permanent RN is unavailable, following the full nursing process.

Responsibilities include home visits for admissions/discharges, teaching family members, and documenting care on portable devices or paper records. CT RN license and 2+ years of acute/medical care experience are preferred.

Qualifications

• Graduate from an accredited School of Nursing required.
• Current Connecticut license to practice as a registered nurse required.
• Two years of recent medical/acute or critical care experience preferred.

Responsibilities

• Provides skilled nursing care in patients' homes, including admission and discharge.
• Teaches and supervises family members and caregivers; documents care accurately.
• Performs nursing tasks per plan of care and ensures confidentiality.
