Job summary

Horizon Home Care and Hospice is seeking experienced nurses to join our Home Care Team as PM Visit Nurses. You will ensure delivery of quality home care services and coordinate care that reflects the standards of home health nursing for assigned patients, providing services at levels consistent with professional practice.

Responsibilities include patient assessments, education, care delivery per physician orders, accurate documentation, and collaboration with the Case Manager and other health

Qualifications

• Assess patient conditions and family/caregiver conditions during each visit.
• Identify and provide for patient and family's educational needs.
• Provide patient care per physician orders and agency policy.
• Complete all required patient and agency paperwork accurately and timely.
• Transcribe, implement and evaluate new physician orders accurately.

Responsibilities

• Work cooperatively with patients, care providers, family members and agency staff.
• Communicate patient's issues and concerns to the Case Manager or Clinical Manager promptly.
