Base pay range

$400.00/daily - $650.00/daily

About The Opportunity

Rockaway Home Care is hiring Per Diem Registered Nurses (RNs) and Outpatient Nurses to support our growing patient population throughout Nassau County, Suffolk County and Queens . Whether you're looking to supplement your income or find a flexible community-based nursing role, we offer a supportive environment and meaningful work in the field.

Key Responsibilities

• Conduct in-home or community-based assessments
• Perform head-to-toe evaluations including medication and diagnosis review
• Complete aide supervisions and provide clinical guidance
• Develop and update personalized Plans of Care (POC)
• Provide orientation to aides based on the POC
• Accurately document findings and maintain compliance with healthcare regulations
  
  
  

Requirements

• Current New York State RN license
• Ability to travel throughout the 5 boroughs, Nassau, and Suffolk
• Medical Professional Liability insurance
• Comfortable using Gmail, Microsoft Word/Excel, and UAS-NY (for home care roles)
  
  
  

Preferred Qualifications

• Bilingual in Spanish/English
• Familiarity with Medicaid, Medicare, or MLTC plans
• Experience in outpatient nursing, home care, or community health
• Proficiency in the UAS-NY platform
• Strong documentation and communication skills
