Qualifications

• Master's degree in Communication Disorders or Speech-Language Pathology.
• Licensed Speech Language Pathologist - MN required if providing services in Minnesota upon hire
• Licensed Speech Language Pathologist - WI required if providing services in Wisconsin upon hire
• Current BLS certification (American Heart Association or American Red Cross) or Allina in-house BLS within 30 days of hire
• Valid Driver's License required; MN/WI residents must obtain license within 60 days of employment if providing services for Home Care and Hospice

Responsibilities

• Provides therapy to patients per physician referral and analyzes patient needs.
• Assesses patients, conducts reassessments, and gathers health data.
• Coordinates, communicates, documents and bills per program, regulatory and professional standards.
• Promotes rehab services and involves patients and families in goal setting.
