Job summary

Bournewood Hospital in Brookline, MA seeks a part-time Social Worker to complete psychosocial assessments for new admissions, develop the initial treatment plan, and support discharge planning.

Responsibilities include conducting evaluations within 48 hours of case assignment, coordinating care with psychiatry, nursing, and rehab teams, and educating families about community resources. Licensure as a Social Worker or eligibility is required; master’s degree is required.

Qualifications

• Masters in Social Work (MSW) required or equivalent.
• Inpatient field placement experience preferred; licensure or licensure-eligibility.
• If licensed, licensure application must be filed with the Board and proof provided to supervisor after appraisal period.

Responsibilities

• Meets with patients to complete psychosocial evaluations within 48 hours of case assignment.
• Assumes role of social worker on the unit; may float to other units as needed.
• Facilitates groups for patients/families and documents participation.
• Communicates findings and progress to treatment team and documents in the medical record.
• Provides consultation to outpatient providers and involved agencies as needed.
• Advocates for and educates family about discharge resources; documents in progress notes.
• Coordinates information/interventions with psychiatry, nursing, rehab and other collateral providers; documents this contact.
