Job summary

Lifeline Home Health, part of the Optum family, seeks a Social Worker - Per Diem in Kentucky. You will provide medical social services under physician direction and collaborate with the Interdisciplinary Group to address social and emotional factors affecting patient health and coping strategies.

Qualifications

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
• Licensed Social Worker in the state of residence (preferred)
• 1+ years of social work experience in a health care setting (preferred)
• CPR certification (preferred)

Responsibilities

• Provide medical social services under the direction of a physician and an Interdisciplinary Group
• Assist in understanding social and emotional factors related to patient health and coping mechanisms
• Engage with patients, families, and communities to support care plans and outcomes
