Qualifications

• Master’s Degree from a school of Social Work accredited by the Council of Social Work
• Licensed as a Social Worker from the State Board of Social Work
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
• 1+ years of social work experience in a health care setting

Responsibilities

• Provides medical social services under the direction of a physician and Interdisciplinary Group to assist in understanding significant social and emotional factors related to health status and development of coping mechanisms
• Advices patients and families to support health goals in a home care setting
• Collaborates with care teams to ensure comprehensive patient care and success in role

Education

Master’s Degree in Social Work (CSWE-accredited)
